I, the undersigned, hereby grant full permission to the Chickasaw Nation its divisions, entities, enterprises, affiliates, agents and their respective officers, directors, employees, advertising and promotional agencies, and anyone authorized by any of them, to use, copy, exhibit, publish, display and distribute my name, portrait, picture, voice, image, photographs, videotaped footage, biographical data and performance, in whole or in part (“my likeness”) in any and all media now known or yet to be developed, including public social media sites (i.e. Twitter, Facebook, YouTube, etc.), for publicity, advertising, marketing or any other lawful uses, without further compensations, notice, consideration, review or consent. I understand that my likeness may be edited and I hereby waive the right to inspect, edit or approve the finished product wherein my likeness appears. There is no time limit on the validity of this release nor is there any geographic limitation on where my likeness may be distributed.

I hereby, for myself, my heirs, executors, administrators and assigns waive and release and agree to hold harmless from any and all rights, claims and causes of action to whatsoever that I may have against the Chickasaw Nation, its divisions, entities, enterprises, subsidiaries, affiliates, agents and its respective officers, directors, and employees for any matter, claim, damage, loss, injury, cause or thing whatsoever arising out of or relating to their use of my likeness.

I hereby affirmatively consent to the jurisdiction of the Chickasaw Nation and the Chickasaw Nation courts for disputes without limit to times and events. I further agree that the laws of the Chickasaw Nation shall govern this Release. Nothing herein shall be construed to waive the sovereign immunity of the Chickasaw Nation, its officers, employees or agents. The Chickasaw Nation hereby expressly retains its full immunity from any unconsented suit.
